Home and Community Care Support Services Toronto Central

Together, we make care happen

Effective April 1, 2021, the Toronto Central LHIN is operating under a new name, Home and Community Care Support Services Toronto Central. Our name has changed but local services and contact information remain the same, including home and community care and long-term care home placement. Health system planning and funding functions are now part of Ontario Health. We’re looking forward to building an even stronger and more integrated health care system together. Home and Community Care Support Services Toronto Central employs over 600 dedicated professionals to serve a unique and diverse population of 1.2 million residents. We work with 24 hospitals, 150 community-based service agencies, 37 long-term care homes, 22 service providers, and 13 community health centres.

Culture

In addition to employee recognition events and awards, we focus on building morale through social activities, feedback forums and the sharing of best practices, while the Work on Wellness Committee reminds team members to take care of themselves. Lunch and Learn events on topics like World Mental Health Day, Black History Month, National Indigenous Peoples’ Day, Asian Heritage Month, Pride Month, and AccessAbility Week, further enhance our commitment to supporting a culture where diversity and inclusion are not a goal but a reality.

Diversity and Inclusion make us stronger

Having a diverse workforce that represents and understands the people we serve within Canada’s most multicultural city is very important to us – as an organization and as individuals. We want all of our employees to feel valued, appreciated, and free to be who they are at work. As a result, we encourage and support all initiatives to build and sustain an inclusive workplace. Our Steering Committee, for example, was created and led by our Black and Indigenous employees to ensure that our policies, procedures and practices are inclusive. Similarly, we established an Employee Experience Committee, a highly diverse team of staff that welcomes all ideas and points of view so that we can provide an accessible working environment.
