Together, we make care happen

Effective April 1, 2021, the Central West LHIN is operating under a new name, Home and Community Care Support Services Central West. Our name has changed but local services and contact information remain the same, including home and community care and long-term care home placement. Health system planning and funding functions are now part of Ontario Health.

Home and Community Care Support Services Central West employs approximately 400 employees who share a patient-centred focus and commitment to provide the best possible patient care. A mosaic of geographic and cultural diversity, and home to over 922,000 local residents, Home and Community Care Support Services Central West deliver and coordinate home and community care for the regions of Brampton, Caledon, Dufferin, Malton, north Etobicoke and west Woodbridge. We work with people of all ages to ensure they can make informed choices about their care, when and where they need it.

Our goal is to acknowledge the richness of our employees’ diverse backgrounds, and to ensure every team member feels appreciated, confident and secure as they engage in their work within the community. Healthy workplace initiatives include an employee/family assistance program, employee leave programs, discounted recreational activities, along with a work-from-home policy to enhance productivity and efficiency.
